<MainHeadline>Text</MainHeadline><Pgraph><Mark1>Objectives and questions: </Mark1>The Ganz acetabular reinforcement ring with a hook (ARRH) was originally developed for acetabular revision in total hip arthroplasty (THA). Beyond its use in revision surgery, the ARRH has demonstrated several advantages as a primary implant, particularly in patients with altered acetabular morphology. The hook enables reliable anatomic positioning by engaging the teardrop, while the ring provides a stable and homogeneous foundation for cementing the polyethylene cup. Owing to these benefits, the ARRH has been extensively used in primary total hip arthroplasty at our institution. The present study evaluates the long-term outcomes of the ARRH with a minimum follow-up of 30 years.</Pgraph><Pgraph><Mark1>Material and methods: </Mark1>This is a retrospective analysis of a total of 208 patients with 240 THAs using the ARRH between April 1987 and December 1991 were retrospectively reviewed after a minimum follow-up of 30 years. 31 of the 240 hips were lost to follow-up, and 157 THAs belonged to patients who had died before 30 years without having undergone revision surgery. Consequently, 52 hips remained available for final evaluation. Of these, 52 hips underwent clinical and radiographic evaluation at a mean follow-up of 33.4 years (range, 30–38 years). In 20 THAs, clinical and radiographic assessment was not performed because revision surgery had been carried out before the follow-up examination. The primary endpoint of the study was revision for aseptic loosening. Level of Evidence B.</Pgraph><Pgraph><Mark1>Results: </Mark1>Out of the 52 hips available for final evaluation, 15 underwent revision for aseptic loosening. An additional 5 hips were revised for deep infection (n = 2), recurrent dislocation (n = 3). The cumulative survival of the ARRH was 0.83 (95% confidence interval 0.75–0.89) at 30 years with any reason for revision. Implant survival with revision for aseptic loosening as the endpoint was 0.87 (95% confidence interval 0.79–0.92) at 30 years. Multivariable analysis identified younger age at surgery as the only independent predictor of achieving a prosthesis age ≥30 years (OR 0.95 per year; p = 0.010). Implant-related variables, including cementation status, acetabular cup size, femoral head size, number of screws, and inlay, were not significantly associated with long-term survival.</Pgraph><Pgraph><Mark1>Discussion and conclusions: </Mark1>The Ganz acetabular reinforcement ring with a hook (ARRH) remains a reliable and versatile option in primary total hip arthroplasty, demonstrating consistent, durable, and long-term survivorship comparable to standard cemented and modern cementless cups. Owing to its structural support and longevity, the ARRH is particularly valuable in cases with altered acetabular morphology, where it provides stable fixation for a cemented cup.</Pgraph></TextBlock>
