      <MainHeadline>Text</MainHeadline><Pgraph><Mark1>Vorgeschichte:</Mark1> Ein 16-j&#228;hriger Junge stellte sich erstmals im Juni 2025 mit R&#252;cken- und Brustschmerzen, Tachydyspnoe, Pr&#228;synkope und Palpitationen vor. Begleitend bestanden Husten, intermittierendes Fieber und vermehrtes Schwitzen.</Pgraph><Pgraph><Mark1>Diagnostik:</Mark1> Laborchemisch zeigte sich eine ausgepr&#228;gte Entz&#252;ndungsreaktion (CRP 22 mg&#47;dl) sowie stark erh&#246;hte kardiale Marker (hs-TroponinI 11.000 ng&#47;l, Norm &#60;45; NT-proBNP 9.000 pg&#47;ml, Norm &#60;400). Im R&#246;ntgen-Thorax fanden sich bilaterale Lungeninfiltrate. Das EKG zeigte T-Negativierungen in II, III, aVL, aVF und V5, die Echokardiographie eine eingeschr&#228;nkte linksventrikul&#228;re Funktion. Im kardialen MRT zeigte sich als Korrelat das Bild einer floriden Myokarditis des linken Ventrikels. In Blutkulturen und serologischen Untersuchungen konnte kein Erreger nachgewiesen werden.</Pgraph><Pgraph><Mark1>Therapie:</Mark1> Es erfolgte eine inotropiesteigernde Therapie mit Milrinon und Levosimendan, sp&#228;ter erg&#228;nzt durch Lisinopril, Bisoprolol und Spironolakton. Leitliniengerecht wurden intraven&#246;se Immunglobuline verabreicht. Zus&#228;tzlich erfolgte eine antibiotische Therapie der Pneumonie.</Pgraph><Pgraph><Mark1>Weiterer Verlauf: </Mark1>Unter Therapie normalisierte sich die linksventrikul&#228;re Funktion, jedoch blieb das Troponin chronisch leicht erh&#246;ht. Sechs Monate sp&#228;ter kam es zu einem Rezidiv mit Thoraxschmerzen. Laborchemisch zeigte sich ein massiver Troponinanstieg auf 20.000 ng&#47;l bei transient erneut eingeschr&#228;nkter Pumpfunktion. Die endomyokardiale Biopsie erbrachte keinen Erregernachweis. Eine erneute Immunglobulingabe f&#252;hrte zu einem weiteren Troponinanstieg. Im Rahmen eines rheumatologischen Konsils fielen ausgepr&#228;gt trockene F&#252;&#223;e sowie Wollhaare auf. Die Kombination aus rezidivierenden myokarditis&#228;hnlichen Episoden, chronisch erh&#246;htem Troponin und kutanen Auff&#228;lligkeiten lie&#223; an eine arrhythmogene Kardiomyopathie aufgrund pathogener Varianten des Desmoplakin-Gens (DSP) denken. Desmoplakin ist Bestandteil des desmosomalen Komplexes und spielt eine zentrale Rolle in der Zelladh&#228;sion von Herz- und Hautgewebe. Pathogene DSP&#8209;Varianten f&#252;hren zu struktureller Instabilit&#228;t, sekund&#228;rer Inflammation und narbigem Umbau mit arrhythmogenem Potenzial. Charakteristisch sind sogenannte &#8222;hot phases&#8220; mit akuter Troponinfreisetzung und entz&#252;ndlicher Infiltration des Herzen. Studien zeigen eine prognostische Verbesserung durch fr&#252;hzeitige, kurzfristige Immunsuppression in diesen Phasen. Daher erhielt der Patient eine Methylprednisolon-Pulstherapie, worunter ein rascher Troponinabfall beobachtet wurde. Die genetische Diagnostik steht aktuell noch aus. Der Fall illustriert das Zusammenspiel einer genetisch bedingten strukturellen Kardiomyopathie mit nichtinfekti&#246;ser Inflammation. Er unterstreicht die Bedeutung einer interdisziplin&#228;ren Zusammenarbeit zwischen Kardiologie und Rheumatologie&#47;Immunologie bei dieser Erkrankung.</Pgraph></TextBlock>
