Es gibt Hinweise darauf, dass das Rauchen die Symptome und den Krankheitsverlauf verschlimmert , . Einige Studien belegen, dass Raucher mit rheumatoider Arthritis einen höheren Bedarf an krankheitsmodifizierenden Antirheumatika (DMARD) haben . Die epidemiologischen Daten zum Rauchverhalten von rheumatologischen Patient*innen sind in den Ländern sehr heterogen , , , , , , , ). Mit dieser Studie wurden die Rauchgewohnheiten und die Motivation zu einer Raucherentwöhnung in Sachsen-Anhalt erfasst.Methoden: Im Rahmen einer monozentrischen Querschnittsstudie in Halle (Saale) mit sukzessiver Rekrutierung füllten die Teilnehmer*innen einmalig einen Online-Fragebogen in LimeSurvey aus. Es wurden Daten zu Alter, Geschlecht, Bundesland, Sozialstatus, rheumatischer Erkrankung, Erkrankungsdauer, Rauchgewohnheiten und Motivation zur Raucherentwöhnung erfasst.Ergebnisse: Für die vorläufige Auswertung lagen 714 vollständige Fragebögen vor. 64,1% der Befragten waren weiblich. Das Durchschnittsalter lag bei 60 Jahren. 26,1% waren aktive Rauchende (weiblich 59,1%), 36% waren ehemalige Rauchende (weiblich 56,4%), 73,9% waren Nichtrauchende (weiblich 65,9%). Von insgesamt 378 Patient*innen mit rheumatoider Arthritis rauchten 31,5% (35,9% der Seropositiven, 23% der Seronegativen). Von 146 Patient*innen mit Spondyloarthropathien waren 29,5% Rauchende. Von 107 Patient*innen mit Vaskulitis oder Polymyalgie waren 7,5% Rauchende. Von 58 Patient*innen mit Kollagenosen waren 19,0% Rauchende. Die Teilnehmerzahlen in den übrigen Gruppen waren zu niedrig, um verlässliche Aussagen zu treffen. In 86,0% der Fälle wurden nur Zigaretten, in 5,4% E-Zigaretten, in 2,7% Tabakerhitzer konsumiert. Eine Kombination von zwei oder drei Produkten gaben 5,9% der Patient*innen an.Von den aktiven Rauchenden hatten 68,1% nicht vor, den Nikotinabusus zu beenden. 6,5% gaben an, in den nächsten vier Wochen und 25,4%, in den nächsten sechs Monaten das Rauchen einzustellen. 12,9% würden an einem Raucherentwöhnungsprogramm teilnehmen wollen. Die bisher am häufigsten genutzten Raucherentwöhnungsmaßnahmen waren die eigene Willenskraft und das soziale Umfeld.Schlussfolgerung: Trotz allgemein bekannter Risiken rauchen 26,1% der Rheuma-Patient*innen. Derzeit gibt es begrenzte Daten für die Auswirkungen von Raucherentwöhnungsmaßnahmen bei Menschen mit rheumatischer Erkrankung. Qualitativ hochwertige randomisierte kontrollierte Studien zur Raucherentwöhnung sind wünschenswert, da die Studienlage zeigt, dass Rauchen einen negativen Einfluss auf die Krankheitsaktivität, die Behandlungserfolge und die Komorbiditäten hat.
